Abstract

Visibility of the facts is a prerequisite for learning from incidents. ECFA+ (Events and Conditional Factors Analysis) is a method that helps investigators structure what they know and what they are uncertain about, in a format that is easy to understand.
This paper provides an overview of ECFA+, its 30-year history, the investigative tasks it is designed to support, and signposts resources freely available from the NRI Foundation. These resources have been newly developed as part of the Foundation¿s mission to further understanding and sharing of knowledge in the field of risk management.
